Admissions & Referrals

MC² Professional Care Services accepts referrals for adult residential mental health placement through Managed Care Organizations, care coordinators, legal guardians, and provider partners. Placement is determined based on eligibility, level of care needs, and current availability.

Who May Submit a Referral

Referrals may be submitted by:

  • Managed Care Organizations (MCOs)

  • Care Coordinators

  • Legal Guardians

  • Hospitals and treatment providers

  • Community provider partners

We do not accept self-admissions without coordination through the appropriate care management process.

Eligibility Overview

Individuals considered for placement must:

  • Be 18 years of age or older

  • Be eligible for Medicaid-funded services

  • Meet criteria for supervised residential support

  • Have an approved service plan coordinated through an MCO

  • Be appropriate for a structured, community-based setting

Final placement decisions are based on clinical appropriateness and program fit.

Referral Process

  1. Referral inquiry is submitted

  2. Relevant documentation is reviewed

  3. A resident screening interview is conducted

  4. Coordination occurs with the care team

  5. Availability and program fit are assessed

  6. Admission date is determined, if approved

MC² Professional Care Services reserves the right to determine appropriateness of placement based on safety, program fit, and regulatory requirements.

Required Documentation

To facilitate review, referrals may require:

  • Current assessment and diagnosis

  • Service plan / ISP

  • Medication list

  • Behavioral history

  • Guardian documentation (if applicable)

Incomplete documentation may delay review.
