MC² Professional Care Services works in collaboration with a network of healthcare providers, care coordination teams, and community partners to support individuals receiving residential mental health services. These partnerships help ensure continuity of care, responsible coordination, and alignment with each resident’s approved service plan.
Our goal is to maintain strong, transparent relationships with organizations that support long-term stability and improved outcomes for the individuals we serve.
Care Coordination
Care coordinators play a central role in supporting individuals receiving residential services. MC² Professional Care Services works closely with care coordination teams to ensure that services remain aligned with each resident’s goals, clinical needs, and approved service plan.
Through ongoing communication and collaboration, care coordinators help facilitate service planning, monitor progress, and support continuity of care.
Community Partnerships
Strong community relationships help support stability and successful integration for the individuals we serve. MC² Professional Care Services works alongside hospitals, community providers, and local support organizations to ensure residents have access to appropriate services and resources.
These partnerships help create a coordinated support network that extends beyond the residential setting.
Clinical & Provider Collaboration
MC² Professional Care Services collaborates with licensed healthcare providers and behavioral health professionals to support residents' ongoing care needs. This includes coordination with prescribing providers, outpatient clinicians, and other healthcare professionals involved in a resident’s treatment plan.
Our team maintains an established relationship with a contracted Nurse Practitioner to support psychiatric medication management and continuity of care when appropriate.
Managed Care Organizations
Residential placements and service coordination are conducted in partnership with Managed Care Organizations responsible for overseeing behavioral health services across North Carolina.
